We are hosting a Sports Physical Night on June 5. Prospective athletes can come to school to get their physical instead of going to the doctors office.
- All physicals performed by licensed medical professionals
- Tickets QR Code Included (need a link to be created)
- Use Door 30 entrance
- Wednesday, June 5th 6-8pm
- Price: $53
- Bring a printed VHSL Physical Form
Sports Athletic Paperwork year 2024-2025
We will no longer be accepting hard copies of the physicals for next year. All paperwork will be scanned and turned in via the activity registry which is required prior to tryouts.
These forms are:
- sports physicals (stamped and dated on or after May 1st),
- Signed certificate of the Concussion Training
- Emergency Blue Card.
