Gainesville Athletics

We would like to communicate a change in the way that we will collect physicals and concussion training paperwork (see below) for the fall sports season. All paperwork will be turned in on a designated date for each sport. Dates and times will be communicated later via Twitter, Facebook, and the Gainesville Athletics website.

Must have:

  • VHSL physical Form (Dated after May 1st 2023 to be valid for 2023-2024 school year)
  • High School PWCS Concussion Training
  • Activity Registration
  • Blue Card
Activity Registration - must be completed prior to tryouts. How to complete activity registration:
  • Go to www.gainesvillecardinals.com 
  • Click on 'Activity Register' on the top left on website
  • Click on the Gainesville Cardinals logo. Below the logo is a "how-to" for parents if you need help.
  • If you have an existing account, click on 'Returning Users', if you are new/transfer, click on 'new users' and follow the instructions. Once account is made and verified, login through 'Returning Users'
    • Once into the account, click on the blue 'register' button on the left.
      • For existing students, click on their name. It will automatically populate information from last year. Please adjust if there is changed in address, medical information, or insurance card information.
      • For new students - click on 'new students' and fill out the information.
      • Make sure to watch the concussion training prior to signing the last step.
      • Click 'submit'. You should receive an email with confirmation that it has been completed.
    • If you do not remember your account, or are having trouble accessing, please contact Janeth Luna or Jason Ritenour for assistance.

Physical Form - Must have all pages filled out and signed. Page 3 must be stamped by Dr.'s office as well. Physicals can be turned in over the summer to the main office for DSA/ Athletic Trainer for review. Must be turned in prior to tryouts. As mentioned above, dates and times for each sport will be communicated at a later time.

  • Go to www.gainesvillecardinals.com 
  • Scroll to 'quick links'
  • Click on 'VHSL physical form'
  • Print and fill out pages 1,2 and 4 yourself, and page 3 will be filled out only by MD,DO, PA or NP, with stamp included.

Concussion training - must be completed After July 1 of the current year (2023). Must be turned in prior to tryouts.

To find Concussion Training:

  • Go to www.gainesvillecardinals.com 
    • Scroll to 'quicklinks'
    • Click on 'online concussion training'
    • Once completed, print out the form and sign both student athlete and parent.

Blue Card:

  • Must be filled out and turned in along with the Physical and Concussion training upon check-in day for your respective sport prior to tryouts.
  • Receive from your respective coach or Ms. Janeth Luna.
